When the response is activated, providers are notified about the outbreak. In this article we describe the multifaceted response we initiated in March 2000 to a syphilis outbreak among MSM. The focus of Los Angeles County's outbreak response plan is collaboration with medical providers and the community at risk. To define the outbreak response, we reviewed all early syphilis (primary, secondary, or early latent) cases reported in Los Angeles County between December 1999 and September 2000 in which the patients were interviewed by disease intervention specialists. 3,4 Among men who have sex with men (MSM), a resurgence of syphilis and other bacterial sexually transmitted diseases has been recently reported in King County, Washington. Almost 30% of the respondents who engaged in sex with chat dates reported inconsistent safe sexual behaviour. Tliis study examined characteristics of men who use gay chat boxes on the Internet, including dating behaviour and sexual risk-taking with sex partners who were initially met through chatting. The questionnaire contained questions on demographics, chatting-related variables, and dating and risk-taking sexual behaviour.  
  
Men one on one sex cam chat boxes were asked to complete a brief questionnaire on the Internet. The results show that a large majority of the 190 respondents reported actual encounters as well as sex with men who were initially met through chatting. The level of unprotected sex increased as the number of sex partners who were met through chatting increased. Cybersex (engaging in sexual self-stimulation while online with another person) and meeting partners for sex are commonly reported among men who have Internet sex with men (MISM). They continue engaging in sexual activities like having sexual intercourse with multiple partners that can lead to health hazards. Collaboration with local health jurisdictions and community providers facilitates cooperative case management and surveillance. Following the normal operating procedures of MSM sentinel surveillance, the sample source analyzed by this paper complied with surveillance program requirements, used a unified survey questionnaire, and regularly reported surveillance data through the National HIV/AIDS data information management system. The MSM sample populations were recruited from local communities. CBOs are recruited to aid in containment efforts and to establish awareness of the outbreak. A media campaign is launched to increase outbreak awareness.  
  
This experience in Los Angeles County with the use of local interventions to respond to an outbreak of syphilis may be useful for other areas of the country at risk for a resurgence of syphilis. The Los Angeles County reactor grid is a surveillance tool used to decide the priority of action for reactive blood specimens, on the basis of a patient's sex, age, and antibody titer. HIV antibodies testing using the method recommended by the WHO: double-testing samples using the serum enzyme-linked immunosorbent assay (ELISA) method for HIV antibody testing. The final method was (3) recruitment via networks, which was conducted throughout the surveillance period. After data were cleaned and unqualified data were excluded, statistical analysis of the surveillance data was reported. Rapid evaluation includes weekly case analysis and case review. Negative results were determined to be negative for HIV and in no need of further review. Negative results were determined to be negative for syphilis and in no need of further review.
